BACK TO CATEGORIES (Click photo to view original size.) Category: Cranes Lifting Devices Hazard: Boom collapse due to inadequate/faulty repair of a damaged boom section. Standard Violated: 29 CFR 1910.180(e)(2)(i) Corrective Action: Ensure that all repairs or modifications are completed by personnel and/or companies qualified to make the repairs and modifications, and that modifications are approved by the manufacture. Perform required inspections and load testing prior to returning the unit to service. Description: The boom of a Manitowoc 3900 crane had been damaged and was repaired by company employees without the approval of the manufacture. Sections of the boom cord were heated and partially straightened and angle iron was welded over the damaged chord sections to strenghten and straighten them. Damaged lacing was cut out and replaced with angle iron. The bottom section of the boom is visibly distorted and bent. |
