Category: Excavations Hazard: Cave-In Hazard: Standard Violated: 29 CFR 1926.652(a)(1) 29 CFR 1926.652(g)(2) Corrective Action: Remove employees from the excavation, widen the bottom of the excavation, place the trench shield within 2 feet of the bottom of the excavation, slope the side walls to 1 1/2 : 1, and have the system approved by a Professional Engineer Description: Employees were working in an excavation 25 feet deep, 30 feet long. and 6 feet wide dug in type "C" soil. The trench shield was 5-7 feet off the bottom of the excavation resting on steps cut into the sidewall. The system had not been approved by a PE.
